This award seeks to recognise a primary care Nurse who has made an outstanding contribution to patient care in their local area. We invite applications from both Nurse’s themselves and nominations from practice colleagues.

Applications should demonstrate:

  • Particular credit will be given to applications with evidence of good leadership and overcoming significant obstacles

Criteria that the judges will be marking against

  • Clear evidence of the Nurse’s impact on their patients, practice or the local area
  • Evidence of exceptional local leadership, imagination and/or innovation and how these qualities have influenced the outcome of their work
