Clinical Team of the Year – Long Term Conditions

There are numerous examples of practice teams whose expertise, professionalism, sensitivity and personalised care have made a huge difference to those suffering with various long-term health conditions. This award seeks to recognise and reward those teams/projects who are dedicated to improving the standard of life for those who are suffering from chronic illnesses in need of constant monitoring and medication such as diabetes, asthma, coronary heart disease, chronic obstructive pulmonary disease (COPD) and mental health issues. Please note we will accept entries that are based around a specific project worked on by a team. We invite nominations from a member of the team or practice colleagues.

Applications should demonstrate:

  • Effective communication of risk factors and symptom with patients
  • Examples of effective team working and staff motivation
  • Innovative programmes to help at risk patients change behaviours
  • Excellent links with partner organisations

Criteria that the judges will be marking against

  • Clearly defined objectives for quality improvement within a project
  • Clearly described interventions
  • Innovation and new ways of working
  • Clarity of leadership and how the qualities have affected the team
  • Quantifiable improvements in patient care, including clinical outcomes
  • The sustainability of the changes observed
  • The ability to be replicated across other practices
